Where do you work?
We are currently serving Davis, Salt Lake, Summit, Tooele, Utah, Wasatch, and Weber counties.
Do I qualify?
We serve seniors ages 55 and older who live in Utah and meet our income requirements.
Do you accept insurance?
We are a Medicaid provider for dental. At this time we cannot bill other insurance providers.
What does my income need to be?
We serve seniors living at or below 200% of the poverty level. These numbers change yearly; check this year's numbers here.
